- Provide your full name, Social Security number, date of birth, and address (must include all addresses you have lived at for the past 5 years).

All identity theft victims should submit a copy of a valid police report as proof of unlawful use of their personal information by another unauthorized person. The report should be completed no later than 90 days before you request a security freeze. This applies to all the CRA’s (credit bureaus).
There are no fees for victims of identity theft, provided they have shown proof of their claim by submitting a valid identity theft report, investigative report, incident report, or complaint with a law enforcement agency about the illegal use of your personal information by another person.
**Experian allows an incident report from the Department of Motor Vehicle (DMV) as a substitute for a police report as proof of being a victim of identity theft.
**Experian doesn’t charge a fee for PIN replacement across all 50 states.

Placing a credit freeze on their credit profile will help stop identity thieves from ruining their credit before adulthood.
Credit bureaus don’t knowingly maintain credit information on minors in their databases. Ideally, there shouldn’t be a file on record, that would mean your child was not victimized.
Some states are requiring that a credit report be created if none exists in order to add a credit freeze. For those states, each credit bureau will create the report and add the freeze at the parent’s request once they receive the required documentation.
You will need to contact each bureau individually by physically mailing the documentation below to place a credit freeze.
- A written request to place a “protected consumer freeze” on the named individual’s file
